Why diagrams are not documentation
A diagram is a picture. A governance record is a structured artefact. These are not the same thing, and treating them as if they are is the source of most documentation failures in enterprise AI.

The governance gap is at the orchestration layer. That is where we need to start.
AI agents are making decisions inside your organisation right now. Whether anyone has formally defined what they are authorised to decide is a different question, and most organisations cannot answer it. The governance gap is not at the policy layer. It is at the orchestration layer, and that is where it has to be closed.
